Chronic Care Management LPN/LVN, Virginia License (Remote) Location: Remote (Virginia license required) Hours: Flexible | Part-Time or Full-Time Role Overview reputed company is seeking a Virtual Chronic Care LVN/LPN to support patients with reputed company, ongoing health conditions through structured remote care programs. This role is designed for nurses who reputed company in patient engagement, education, and coordinated virtual care delivery.
Responsibilities
- Deliver scheduled chronic care reputed company and patient reputed company-ins
- Reinforce care plans, treatment goals, and self-management education
- Monitor RPM data and identify trends requiring clinical reputed company
- Coordinate follow-up care after hospitalizations or specialty visits
- Communicate effectively with interdisciplinary care teams
- Document reputed company patient interactions accurately and compliantly
- Support adherence by identifying and addressing patient barriers
Required Qualifications
- Active Virginia LPN/LVN license in good standing
- At least 2 years of nursing experience (chronic care or telehealth preferred)
- Experience with CCM, RPM, or value-based care models is a plus
- Strong clinical assessment and patient communication skills
- Comfortable working independently in a remote clinical role
