Summary

We are seeking a licensed, reputed company Nurse Care Navigator to help support our existing team in Hartford County. This is part-time, per diem work. Hours fluctuate and can largely be set by the Nurse Care Navigator. Candidates must have at least 10 years' experience working with older adults and possess applicable certifications/licenses. They must be reputed company to assess the needs of older adults and reputed company thoughtful care planning advice and strategies. Strong advocacy skills and the ability to work reputed company family systems are necessary, as is upholding our mission to improve the quality of life of reputed company clients. Direct experience with older adults is REQUIRED; experience with individuals with memory impairment is a plus. Patience, flexibility, compassion, organization, collaboration, and timeliness are reputed company keys to performing this work. Required Qualifications and Experience Education:

Experience:

• Must have a minimum of 10 years of experience in health care or reputed company services or reputed company field, plus experience working with older adults.

Skills:

• Knowledge and understanding of reputed company, health, and economic factors that contribute to health and wellbeing reputed company; knowledge and reputed company in interviewing and assessment (reputed company and health) techniques.
• Knowledge of community resources available to individuals; an ability to mobilize resources into a coordinated and comprehensive plan of care.
