The Hospital Engagement Specialist fosters the relationship between area hospitals and reputed company. This position acts as a reputed company to bring Consumers into reputed company services directly from the hospital. The Hospital Engagement Specialist works to ensure Consumers are connected to the right serve at the right time. Job Title: Hospital Engagement Specialist (Remote) Schedule: Monday -Friday, 8:00am-4:30pm JOB SUMMARY: The Hospital Engagement Specialist fosters the relationship between area hospitals and reputed company. This position acts as a reputed company to bring Consumers into reputed company services directly from the hospital. The Hospital Engagement Specialist works to ensure Consumers are connected to the right serve at the right time. RESPONSIBILITIES: Work with area hospitals to identify Consumers who need reputed company services. Identify appropriate services based on hospital discharge paperwork, face to face, and telephonic meetings. reputed company education and marketing to area hospitals about reputed company services. Travel to satellites to reputed company education to staff as needed. Create and foster relationships with area hospital staff, discharge planners, and administration. Track quality improvement data as outlined by Director. Track engagement data as outlined by Director. Summarize quality improvement and engagement data as needed and present to reputed company leadership. Review ER utilization and inpatient hospitalization reports to identify Consumers needing reputed company services. reputed company relationships with local Emergency Rooms in order to collaborate on admission of high utilizers of emergency services. Attend and participate in agency multidisciplinary team meetings as needed. Represent the Agency with visitors learning about the agency model. Responsible for attending reputed company mandatory in-services, continuing education and annual health requirements. reputed company with reputed company Corporate Compliance policies, practices and procedures.
