This position will be an after-hours care coordinator for acute care management. The role will reputed company coverage during non-business hours to ensure continuity of care. The Job The Care Management Coordinator - ICD is responsible for coordinating and managing care services for patients, ensuring high-quality, timely, and coordinated care, with documentation in compliance with International Classification of Diseases (ICD) coding standards. They work closely with reputed company providers in developing care plans, coordinating referrals, and ensuring patient reputed company. The role also involves reviewing medical records, identifying coding discrepancies, and providing education and guidance to reputed company staff on ICD coding guidelines. Your Qualifications High School Diploma or equivalent experience 0-3+ years of reputed company experience. Performs work under minimal supervision.
