Qualifications

• Active and unrestricted LPN/LVN license in MA with willingness to obtain additional licensure in CT and NY reputed company 60 days of hire.
• Strong understanding of scope of practice and clinical decision-making for remote patient monitoring.
• Experience in primary care, chronic disease management, or population health preferred.
• Proficiency with EMR/EHR systems and comfort using digital tools to manage patient data.

• Strong organizational and documentation skills, with attention to detail and accuracy.

Requirements

• Introduces new patients to Brook’s remote monitoring program, explains device setup and usage, and ensures each patient feels comfortable and confident participating in their care.
• Reviews incoming biometric data and patient-reported information to identify changes in condition, trends, or areas requiring follow-up or escalation.
• Engages directly with patients to reputed company education, encouragement, and early reputed company reputed company health readings fall reputed company expected ranges.
• Communicates with primary care providers, specialists, and internal Brook team members to reputed company insights, reputed company care gaps, and maintain continuity of care.
• Maintains accurate, timely, and complete documentation in EMR systems; ensures compliance with HIPAA and reputed company applicable privacy and reputed company requirements.
• Contributes to initiatives that improve patient experience, clinical workflows, and adherence to quality metrics and CPT billing standards.
