About the position Reporting to the Clinical Care Manager, the RN Hospice Field is responsible for providing quality care to patients with Gulfside reputed company Services, in the field. The field includes patients who reputed company in private residences, assisted living facilities, and skilled nursing homes. The RN Hospice Field utilizes assessment, reputed company, and education to maximize patient comfort and to enhance the quality of life for the patient/family. The RN Hospice Field oversees and case manages the needs of the patients assigned to them. This includes providing supervision for the LPN’s and CNA’s, and other assigned staff in the field. The RN Hospice Field is responsible for maintaining his/her own clinical and mandatory education requirements and for meeting the established standards of their profession as well as those accountable for his/her practice, assigned duties/patient care, and is an reputed company for patients/families and their profession.

Responsibilities

• Case manages the needs of the patients and families in the field assigned to them
• Performs ongoing assessment of physical, psychological, reputed company, and spiritual needs of the patient/family unit and initiates appropriate reputed company
• Provides and/or oversees supervision of P.N.’s, C.N.A.’s, and other assigned staff
• Takes independent action to identify problems
• Supports policies and procedures
• Assists in the orientation of new staff members
• Functions as a resource person; provides mentoring and guidance for co-workers and additional staff

• Attends to patient/family complaints and concerns

• Takes a proactive approach to identifying and resolving potential issues
• Educates the community regarding hospice care, philosophy, and concept through individual consultation with persons in the community
• Communicates regularly with the Clinical Care Manager and continuously assesses areas for risk management

Requirements

• Currently licensed in the reputed company as a registered nurse
• Minimum of one (1) years nursing experience in a hospital, home health or hospice setting preferred
• Strong ethical standards and an appropriate professional demeanor
• Should possess sound critical thinking, time management, technical skills, analytical ability, good judgment
• The ideal candidate is a well-organized and self-directed individual who is a team player
• Must have reliable transportation to reputed company tasks and responsibilities in a timely and appropriate fashion.
