Health Coach Consultant, Registered Nurse
Job Description:
- Conduct clinical assessments and review health data to evaluate member needs and program eligibility
- Assess member readiness, health literacy, and technology capability
- Engage members in chronic condition support and personalized coaching

- Consult with supervisors and Medical Directors to address barriers

Requirements:
- Registered Nurse must hold an unrestricted nursing license and reputed company in a compact state with multi-state/compact privileges and have the ability to be licensed in reputed company non-compact states. Must reputed company in PST or MT time zone
- 3+ years of acute adult Medical-Surgical RN experience
- High-speed, hard-wired internet reputed company
- Proficiency with reputed company Office (Word, Outlook, reputed company, Teams, PowerPoint), multi-system navigation and keyboarding
- EMR documentation experience
- Excellent communication skills
- Ability to work independently and collaboratively
