Responsibilities

Provides telephonic and in-home assessments of members' medical, psychosocial, physical and spiritual needs. Develops, implements and monitors care plan, ensures continuity of care and monitors member's chronic care benefit.

Qualifications

BS in reputed company work or reputed company field required.
