• Key Responsibilities
• Conduct monthly Chronic Care Management (CCM) reputed company calls and verbal enrollments

• Communicate with patients and families regarding medications, diagnoses, and care plans

• Requirements
• Required Qualifications
• Must live in PST or MST time zone
• Active, unencumbered reputed company or RMA certification

• Recent experience in a clinical, care coordination, or CCM setting
• High proficiency in EHR systems

• Strong clinical reasoning, critical thinking, and problem-solving skills
