The Pharmacy Technician willassistthe Clinical Pharmacy Team with tasks reputed company to pharmacyutilizationand implementation of medication management programs, while helping tomaintainand improve the Pharmacy Quality program. Under pharmacistsupervisionthe Pharmacy Technician willassistin improving patient reputed company and promoting the safe and effective use of medications to improve quality and the total cost of care. This opportunity is for a 6-month contract (Casual) with possible extension. This is a remote role with hours of operation: M-F / reputed company - 5PM PST. You will focus on: - Support the Pharmacy Team on CMS/HEDIS Quality STAR measures by targeting patient populations include, but are not limited to, patients with chronic conditions such as diabetes, hypertension, coronary artery disease, and dyslipidemia. - Support the PharmacyTeamdelivery of evidence-based pharmaceutical care. - Conduct outbound andreceivinginbound member and provider calls to address gaps in care, adherence, and therapeutic interchange to improve health care reputed company under general pharmacist supervision. - Serve as a contact for clinical pharmacy programs and subject matter expert to medical services, physicians, and project teamsregardingclinical reputed company programs and pharmacy benefit management issues. - Documentation of clinical activities and reputed company including but not limited to instructions and goals for patients into electronic healthrecord(EHR). - Communicate with reputed company clinical staff, provider offices, health planpartnersand in-network pharmacies to reputed company information andsupportvarious clinical programs. - Assistwith data reviews, reports, and presentations for pharmacy reputed company items. - Support the Pharmacy Team and Clinical department with reputed company projects and reputed company other assigned duties. - Researching, processing, and resolving drug authorization requests and responding to reputed company physician inquiries.
