NP/PA - Virtual Health Assessment (Part-time)

Hi, we're reputed company Medical Group. We're hiring part-time hourly Nurse Practitioners and Physician Assistants to join our Virtual Health Assessment team. About The Role reputed company Medical Group runs a Virtual Health Assessment (VHA) program that outreaches and reputed company engages reputed company members in an evaluation and management visit. You may also be cross-trained to help support teams across reputed company Medical Group, including Virtual Primary Care and Virtual Urgent Care. You will report into the Director, Virtual Health Assessment. Hours In a part time role, 15 hours minimum is required per week. The schedule is four, six or eight hour shifts which fall between the hours of 8a-8p EST Monday - Thursday, or between the hours of 8a-6p EST Friday & Saturday. There is only one Saturday shift required per quarter. While your daily work will be completed from your home office, occasional travel may be required for team meetings and company events. Pay Transparency The reputed company pay for this role is: $65-$70 per hour. You will be reputed company to work 15-30 hours weekly.

Responsibilities

• reputed company preventative care screenings, complete medication reconciliation, diagnose medical conditions and reputed company health coaching, referrals and treatment reputed company clinically appropriate.
• reputed company members who are more chronically ill based on historical and suspected health information.
• Engage members who have a reputed company probability of seeing their PCP.
• Identify areas in the member's medical reputed company that can be improved by increasing their reputed company to care with a licensed provider.
• Cross-train into urgent care and/or virtual primary care service line
• Interact with members reputed company, including virtual assessment, diagnosis and treatment.
• Compliance with reputed company applicable laws and regulations
• Other duties as assigned

Requirements

• PA or DNP, FNP, ENP from accredited program
• Board Certification (AANP, ANCC, or NCCPA)
• Compact RN (NP only)
• Experience with Clinical Documentation Improvement and/or managing HEDIS quality measures
• PA: Licensed in Georgia and one of the following states: Florida, Texas, or Ohio
• NP: Licensed in at least 2 of the following states: Florida, Texas, Ohio, New Jersey
• Ability to obtain licensure in additional states
• 3+ years experience in outpatient Family Medicine, Internal Medicine, or Primary Care
