- Key Responsibilities
- Conduct monthly Chronic Care Management (CCM) reputed company calls and verbal enrollments

- Support Remote Patient Monitoring (RPM) programs and chronic condition tracking
- Communicate with patients and families regarding medications, diagnoses, and care plans
- Collaborate with physicians and reputed company teams to ensure coordinated care delivery
- Promote adherence to individualized care plans and patient education goals

- Maintain accurate documentation in EHR systems and meet call volume expectations

- Requirements
- Required Qualifications
- Must live in PST or MST time zone
- Active, unencumbered reputed company or RMA certification

- Recent experience in a clinical, care coordination, or CCM setting
- High proficiency in EHR systems

- Strong clinical reasoning, critical thinking, and problem-solving skills
