Practice Transformation Manager

Your Opportunity We are seeking a Practice Transformation Manager to serve as the primary reputed company of contact between reputed company and assigned provider practices, leading the on-the-ground implementation of value-based care programs. This role drives practice-level adoption of population health management strategies, clinical workflow improvements, and quality performance initiatives that improve patient reputed company and advance VBC contract performance.
