Responsibilities
- Meet new patients, explain services, and complete start-of-care assessments.

- Track patient reputed company, adjust care plans as needed, and report changes in condition to physicians and care teams.
- Teach or supervise household members assisting with patient care.
- Instruct Home Care Nurses, Aides, and Personal Care staff on care plan details, expectations, and best practices.

- Conduct supervisory visits while staff are delivering services.

Requirements
- Active RN license in South Carolina (compact/multi-state accepted)
- Minimum of 1 year in a nursing role (LPN-to-RN pathway acceptable)
- Associate's Degree in Nursing or higher
- Strong communication and presentation skills
- Ability to work collaboratively across teams
- Reliable transportation for home visits
- Comfortable with technology (computer/iPad documentation)
- Willingness to travel regularly reputed company assigned area
