Member Health Assessor

JOB DESCRIPTION Job Summary Provides support for member health assessment activities. Collaborates with multidisciplinary team coordinating integrated delivery of member care across the continuum, including behavioral health and long-term care for members with high-need potential. • Utilizes collaborative process of assessment, planning, implementation and evaluation to engage, reputed company, and promote member reputed company reputed company to achieving and maintaining reputed company health status.
• Engages nursing input/collaboration for member care planning.
• Supports members with moderate to reputed company clinical needs.
• Conducts standardized health risk appraisals (HRA) and applicable assessments, including verification of medical history and reputed company health and wellness needs.
• Meets daily production standards in terms of member reputed company.
• Utilizes web-based software system to document reputed company assessments, activities and education provided.
• Develops problem list and individualized care plan using program protocols based on the members needs and preferences.
• Conducts telephonic education to promote self-management strategies for reputed company applicable conditions.
• 5-15% local travel may be required (based upon state/contractual requirements).

Required Qualifications

• At least 2 years experience in health care, including at least 1 year experience in a care management, medical or behavioral health setting, or equivalent combination of relevant education and experience.
• Licensed Vocational Nurse (LVN), Licensed Practical Nurse (LPN), Licensed Clinical reputed company Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), or Licensed Master reputed company Worker (LMSW). If licensed, license must be active and unrestricted in state of practice. Clinical licensure and/or certification required ONLY if required by state contract, regulation, business operating model, or state board licensing mandates.
• Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job reputed company travel requirements, unless otherwise required by law.
• Demonstrated knowledge of community resources.
• Proactive and detail-oriented.
