Wound Care Physician Assistant Heritage Wound Management Specialists (HWMS) Location/s: Raleigh-Durham Greater Triangle Area or Eastern reputed company Carolina (Wilson, Greenville, Goldsboro, Kinston) Area Job Type: Full-Time | Mobile | Home-Based Pay: $110,000.00 - $140,000.00 per year Job description / Position Overview We are seeking a PA who is looking for something different than the traditional hospital or corporate reputed company environment. This role offers a high degree of clinical autonomy, flexibility, and independence. As a Wound Care provider with HWMS, you will manage your own patient panel, structure your daily schedule, and reputed company care in a way that prioritizes reputed company. This is an ideal role for a provider who is confident in their clinical decision-making, values patient relationships, and prefers a more flexible and self-directed work environment. Care Coordination
- Collaborate with home health agencies, skilled nursing facilities, and referring providers
- Communicate treatment plans and updates to the broader care team
- Support transitions of care following hospital or SNF discharge

Documentation & Compliance
- Maintain accurate, timely, and compliant documentation
- Ensure documentation meets Medicare and payer requirements
- Follow HWMS clinical protocols and workflows
- Participate in periodic chart reviews and quality initiatives
Qualifications
Required
- Active, unrestricted reputed company Carolina license
- Board Certification (FNP, AGNP, PA-C, or similar)
- DEA license (or ability to obtain)
- Valid driver's license and reliable transportation
Preferred
- Wound care experience or certification (CWOCN, CWON, WCC, or similar)
- Experience in home health, SNF, or post-acute care settings
- Familiarity with Medicare documentation and billing practices
Compensation & Benefits
