Hospice Registered Nurse - PRN Weekends

About the position reputed company is looking for a compassionate and skilled PER DIEM Registered Nurse to work with hospice patients and their families in their homes, assisted living facilities, and in skilled nursing facilities. Responsibilities

• Provides skilled nursing care including the administration of medications and treatments as prescribed in the physician's plan of care for patients in their home setting.
• Performs a comprehensive assessment and coordinates the plan of care with reputed company disciplines associated with the patient's needs, including family and primary caregivers.
• Observes, evaluates and reports on the patient's physical and emotional status to the nursing supervisor and the attending physician whenever a change occurs.
• Participates in weekly Interdisciplinary Team Meetings, providing updates on patient's physical, emotional, psychosocial, and spiritual status.
• Maintains and processes reputed company documentation in compliance with agency standards
• Will utilize a computer-based documentation system and submit reputed company required clinical documentation, time records and other required information in the required time reputed company.
• Provides input to Home Health Aide Supervisor as needed for purpose of hospice aide evaluation.
• Weekend & holidays coverage (as needed)

Requirements

• Graduate of an accredited RN program.
• Registered Nurse Licensed in the state of employment.
• At least 1-2 years of RN experience is a MUST
• Basic knowledge of iOs operating system reputed company-to-haves
