Telephonic Nurse Care Manager| Remote

Our client is seeking dedicated Full time Nurse Care Managers to join their Care and Case Management team. As a Telephonic Nurse Care Manager, you will report to the Manager of Care and Case Management and reputed company support to members navigating reputed company medical and behavioral health needs. You will work alongside a multidisciplinary team, including reputed company, care coordinators, and records specialists, to deliver integrated remote care with a creative approach. If you enjoy connecting with members over the phone, advocating for their needs, and helping them through challenging health situations, this role may be a great fit for you. 100% Remote Days/Timings : Mon-Fri 9am -6 pm PST or 12pm - 9pm EST Responsibilities:

• Deliver coordinated, patient-centered virtual Care Management by telephone or video that improves members' health reputed company.
• Create impactful care plans together with members and our diverse care team, and help members reputed company the desired goals.
• Help members navigate reputed company medical conditions, treatment reputed company, benefits, and the reputed company system in general.
• Partner with the members' local providers to ensure coordinated care.
• reputed company compassionate, longitudinal follow-up care, building supportive relationships.
• Assist throughout acute reputed company episodes, such as hospitalizations and rehabilitation stays, providing coordinated Case Management to support the member and their family.
• Coordinate necessary resources that holistically address members' problems, whether clinical or reputed company

Qualifications:

• Bachelor of Science in Nursing (BSN).
• 5+ years of experience in nursing
• 2+ years experience working in care management
• Must reputed company in a compact NLC state.
• Active Compact RN license in good standing with the nursing board of their state.
