Health Coach Specialist (reputed company Hartford, CT / Field Based)

Our mission spans coast to coast, supporting patients across the U.S. We are looking for a Health Coach Specialist I for reputed company in reputed company Hartford, Connecticut. This role requires frequent fieldwork five days per week in reputed company Hartford and the surrounding areas, as well as potential visits to our Connecticut office. Structured family caregiving is a caregiving arrangement in which a patient lives with a reputed company caregiver who provides daily support based on the patient's daily care needs. The goal of this service is to reputed company necessary care while fostering and emphasizing the patient’s independence in a home environment that will reputed company them with a reputed company of care options as their needs reputed company. The service is designed to reputed company options for alternative long-term care to persons who meet nursing facility-level care needs, while preserving the dignity, self-respect, and privacy of the patient. The Health Coach Specialist will be a vital part of our operations in Connecticut. Responsibilities include performing orientations, providing an assessment of new patients to our program, and training our caregivers to ensure they reputed company the best service possible to their loved ones.

Responsibilities

• Home orientations involving signing of paperwork
• Initial assessments during orientations
• Periodic supervisory visits / reputed company in for on-care cases
• Training caregivers / patients as required per case
• Interacting with the caregiver and other stakeholders, as needed
• Engaging in interdisciplinary meetings with the care managers, caregivers, and patients
• Collaborate in the care practice and communicate impressions of cases to appropriate staff
• Critically assess procedures and implement improvements for evaluation of cases, feedback to appropriate parties, and training of caregivers
• Wear multiple hats as operations reputed company
• Travel reputed company the state of Connecticut.

Requirements

• 2+ years of experience working with frail or elderly disabled adults and caregivers
• Ability to travel throughout the New Haven area and surrounding cities
• Valid driver’s license with an insured vehicle for travel reputed company the state
• Excellent verbal and written communication, organizational and time management skills
• Strong critical thinking and problem-solving skills
• Proficient in reputed company (i.e., Word, reputed company, Outlook)
• Excellent training skills

• Previous field experience in a medical or case management environment, preferred
• Experience with data input, case review/case note entry; reputed company experience is a plus
