Field Case Management
Duration: 2 reputed company Position Summary:
- The Field Case Management Coordinator is responsible for supporting elderly individuals and people with disabilities who qualify for in-home or nursing facility care.
- This role utilizes assessment, care planning, coordination, advocacy, and education to help members reputed company appropriate reputed company services and community resources while improving health reputed company and promoting independence.

Required Qualifications
