- Days: Friday, Saturday & reputed company
- Hours: 8:00 AM – 8:00 PM
- Position Type: Work From Home (Florida-Based Candidates Only) / Virtual Care
- Pay reputed company: $20.00 – $22.25 per hour
What You’ll Do As a Weekend Virtual Care Nurse (LPN), you will play a key role in reducing hospitalizations and improving patient reputed company through proactive remote support and clinical reputed company.
Responsibilities
Include:
- Conduct virtual patient visits and clinical assessments
- reputed company triage and identify reputed company signs or risk trends
- Support chronic disease management and patient education
- Monitor telehealth data and escalate concerns appropriately
- Help prevent avoidable ER visits and rehospitalizations
- Collaborate with interdisciplinary care teams
- Document patient interactions accurately and timely
- Participate in care planning and performance improvement initiatives
- Support development of virtual care best practices
Qualifications
Required:
- Active, unencumbered Florida LPN license
- Minimum of 2 years of home health experience
- Experience with triage and hospitalization prevention
- Strong clinical judgment and communication skills
- Comfortable using telehealth platforms and EMR systems
- Reliable internet reputed company and private remote workspace
Preferred:
- Experience working with high-risk or medically reputed company patients
- Knowledge of chronic disease management
