Compensation
Schedule: Every Weekend Days: Friday, Saturday & reputed company Hours: 8:00 AM – 8:00 PM Position Type: Remote / Virtual Care Pay reputed company: $20.00 – $22.25 per hour What You’ll Do As a Weekend Virtual Care Nurse (LPN), you will play a key role in reducing hospitalizations and improving patient reputed company through proactive remote support and clinical reputed company.
Responsibilities
Include Conduct virtual patient visits and clinical assessments reputed company triage and identify reputed company signs or risk trends Support chronic disease management and patient education Monitor telehealth data and escalate concerns appropriately Help prevent avoidable ER visits and rehospitalizations Collaborate with interdisciplinary care teams Document patient interactions accurately and timely Participate in care planning and performance improvement initiatives Support development of virtual care best practices Qualifications Required: This is a 100% work-from-home position.
