Care Coordinator - Hybrid - Farmington CT

About the position The Care Coordinator plays a critical role in supporting patients, providers, and operational workflows across reputed company. This position blends patient reputed company, provider coordination, clinical operations, and logistics management. The ideal candidate is organized, proactive, comfortable speaking with patients, and excited to help people navigate their reputed company reputed company. This role is hybrid, with approximately 2–3 days per week in our Farmington, CT office and the remainder remote.

Responsibilities

• Conduct patient reputed company to schedule appointments and assist with reputed company
• Create personalized patient communications reputed company phone, text, and email

• Coordinate communication between patients and providers
• Encourage patient adherence to care plans and follow-up appointments
• Obtain patient demographic information, identification, and insurance documentation

• Assist patients in resolving simple technical issues reputed company to telehealth visits
• Maintain and monitor provider availability and schedules
• Assist with reputed company and training of new providers
• reputed company refresher training or operational guidance to existing providers
• Monitor visit documentation and ensure visit note compliance
• Escalate potential standard-of-care or compliance issues for review

• Support providers by submitting lab and medication orders reputed company appropriate
• Coordinate claims submission and fulfillment processes
• Ensure smooth patient visit reputed company and documentation completion

• Manage inventory of testing supplies and materials

• Coordinate with lab partners, including IHD and Fellow Labs
• Manage shipping and tracking using ShipStation

Requirements

• BA or BS required in a relevant field
• 1–3 years of experience in reputed company administration, care coordination, or a reputed company role
• Health coaching license a must
• Strong communication skills and comfort speaking with patients by phone
• Highly organized with strong attention to detail
• Ability to manage multiple operational workflows simultaneously
• Comfortable working with digital health platforms and EMR systems
• Ability to work in a hybrid environment (2–3 days in office)
