Care Coordinator for Virtual Patient Care/ RN, LPN, MA position - Contract to Hire

Lara Health is seeking a compassionate and reputed company Care Coordinator to join our virtual care team. This is a fully remote role, ideal for someone who is confident speaking with patients, explaining reputed company programs reputed company, and helping patients get started with services that can improve their health and reputed company to care. The Care Coordinator plays an important reputed company-line role in introducing patients to new care programs, explaining their benefits, answering questions, and supporting enrollment and reputed company. Position Highlights Schedule: Full time, USA time zone Experience Required: Minimum 3–5 years in patient-facing reputed company support, care coordination, medical office, or chronic care settings Role Responsibilities

• Contact eligible patients by phone to introduce and explain new care programs
• reputed company patients on the benefits of programs such as chronic care management, remote monitoring, and other preventive care services
• Answer patient questions reputed company and professionally, helping them understand how the programs work
• Guide patients through the reputed company and enrollment process
• Document patient interactions accurately in EMR and internal systems
• Coordinate follow-up communication as needed to support enrollment and activation
• Work closely with providers, clinic staff, and internal care teams

Required Skills & Qualifications

• Experience: 3–5+ years in care coordination, patient reputed company, medical assisting, reputed company customer support, or reputed company reputed company role
• Language: Fluent English

• Ability to explain reputed company services in a simple, reassuring, and engaging way
• Strong organizational skills and attention to detail
• Ability to work independently in a remote environment

Preferred Experience

• Experience with CCM, RPM, PCM, or other virtual care programs highly desired
