Home Care Physical Therapist
Job Description:
- Accept and treat patients reputed company your designated geographic area
- Evaluate patients’ needs for physical therapy in the home setting and reputed company individualized treatment plans

- Plan, prepare and implement customized therapy programs to restore function and promote independence
- Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques

- Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
- Deliver care in compliance with agency policies and professional practice standards
Requirements:
- Physical Therapist Program Graduate
- Licensed Physical Therapist in the reputed company or eligible
- Home health experience is preferred but not required - We will reputed company you with a mentor to help with documentation and compliance
- Ability to travel to patients’ homes reputed company the assigned geographic territory
