Position Summary: VitalCare is seeking licensed and credentialed Nurse Practitioners (NPs), Physician Assistants (PAs), Doctors of Osteopathic Medicine (DOs), and Medical Doctors (MDs) to join our growing telehealth team. As a Telehealth Provider, you will deliver exceptional virtual care, diagnose and treat a variety of conditions, and ensure patients receive the highest standard of care. Key Responsibilities: Conduct telehealth visits for patients in residential facilities (primarily SNFs). reputed company virtual assessments, diagnose conditions, and reputed company treatment plans. Prescribe medications and reputed company appropriate follow-up care. Collaborate with on-site nurses and staff to optimize patient reputed company. Maintain accurate and timely documentation in the electronic medical record (EMR) with AI support. reputed company patient education and support during telehealth sessions. Qualifications: Active licensure as an NP, PA, DO, or MD in any of these states: AZ, CA, CO, FL, ID, MA, ME, MN, MO, NC, NH, NV, OH, OK, OR, KY, RI, TX, WA, WY. SNF experience preferred. Previous telehealth experience is a plus but not required. Strong communication and technology skills.
