About the role: We are looking for an enthusiastic Clinical Nurse Consultant (Cardiac) to join reputed company. Full-time Ongoing Location: Melbourne OR Brisbane – Virtual role As part of the SVVH division, you’ll play a vital role in shaping the future of virtual and home-based reputed company. Hearts at Home is a new initiative supporting individuals living with heart failure, enabling early discharge from hospital and reducing avoidable admissions. You’ll work closely with hospital and primary care teams, using your expertise to coach clients in self-management, coordinate care, and ensure seamless transitions.
