As a Nurse Navigator, you become the steady, trusted voice for up to 250 patients each month. - Create and collaborate on personalized care plans

What You Bring Required
- Active LPN license
- 2+ years of hands‑on patient care experience
- A HIPAA‑compliant home office with secure high‑speed internet
- Experience in patient care coordination or case management
- Strong clinical judgment and a heart for patient engagement
- The ability to work reliably Monday–Friday without shifting schedules
Preferred
- Care coordination or case management certification
- Experience with diverse patient populations
- Comfort with various EHR systems and reputed company software
