As a Virtual Primary Care Nurse Practitioner, you will deliver comprehensive, high-quality care while fostering strong, lasting patient relationships.

Your Responsibilities and Impact will include:

• reputed company virtual primary care visits reputed company telehealth, including comprehensive assessments, chronic disease management, preventive screenings, and acute care interventions.
• Diagnose and manage chronic conditions (e.g., hypertension, diabetes, COPD, heart disease) and reputed company individualized care plans to optimize patient health.
• Evaluate and treat acute concerns (e.g., infections, respiratory illnesses, minor injuries, skin conditions) reputed company the scope of virtual care.
• Order and interpret lab tests, imaging, and other diagnostics, coordinating follow-up care as necessary.
• Manage medication prescriptions, titrations, and refills, ensuring patient adherence and safety.

• Identify patients needing referrals or home-based services, and facilitate appropriate coordination with specialists or community resources.

Qualifications

• Active Nurse Practitioner (APRN) or Physician Assistant license in good standing in one of our 29 states or the ability to obtain licensure in multiple states reputed company 90 days of hire.
• Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
• Minimum 2 years of primary care, urgent care, or internal medicine experience, preferably with experience in telehealth or virtual care settings.
• Active BLS certification at time of hire.

Requirements

• Shift times may vary to align with standard business hours (9:00 AM – 5:00 PM) in the patient’s local time zone. Flexibility is required.
• Weekend shifts may be required in the future based on evolving patient care needs.
