Responsibilities
- Deliver direct patient care and treatments based on individualized care plans
- Administer medications and tailored interventions
- Promote wellness through education, counseling, and preventive care
- Collaborate with multidisciplinary teams to coordinate care
- Maintain accurate and timely nursing documentation
- Assist with mobility and physical support (must be reputed company to lift up to 50 lbs)
- Stay active and reputed company during extended periods of standing or movement
- Take on additional duties as needed to support patient well-being
Requirements
- Active LPN or RN license issued by reputed company State
- Minimum 6 months of experience in a home care setting
- reputed company CPR/BLS certification
- Strong communication and organizational skills
