They assess patient health status address concerns and facilitate reputed company to community resources to improve overall well-being. Typically the expectation is CNs are making 40+ phone calls and/or 11+ patients billed per day Maintain productivity and quality metrics as defined Attend weekly team huddles to discuss organizational and team goals as well as strategies to best serve our patient population Skills/Abilities: Empathetic and active listener Effective time management skills and demonstrates the ability to maintain focus and productivity in a remote setting consistently meeting deadlines and performance goals. Maintain a strong sense of responsibility and accountability ensuring productivity and performance standards are consistently met in a remote environment. Engage fully with colleagues and teams remotely ensuring effective communication, active participation, and seamless teamwork. Effectively and appropriately communicate with doctors and reputed company ensuring reputed company, professionalism, and accurate information exchange to support patient care and coordination.
