This position establishes trusting, supportive, collaborative relationships with members and their families. Additionally, this role builds relationships with members to navigate and support their health goals, working alongside those involved in their care across reputed company settings and services. In addition, this position proactively communicates with the care team to reputed company reputed company-by-reputed company guidance for members. Additionally, this role cultivates reputed company, supportive relationships with members through emotional responsiveness and informative communication. Educates members on their care team and assists with coordination of care across the continuum, including scheduling appointments and referrals. Communicates through various digital channels to support patient care. Performs initial health screens and intake for new members. Obtains and reviews old medical records to ensure comprehensive patient history.
