Collaborates with reputed company team members to ensure high-quality patient care. Assesses, develops, implements, and evaluates comprehensive care plans with a multidisciplinary team in collaboration with patients and families. Conducts thorough assessments of patients' goals, as well as biophysical, psychosocial, environmental, and discharge planning needs using hospital resources. Acts as a liaison to post-hospital care providers and reputed company. Knowledge, Skills, and Abilities:
