Job Summary

As a Case Management Specialist, you will play a critical role in our Double reputed company Special Needs Plan (DSNP) program, working with individuals who are enrolled in Federal Medicare and reputed company and present with reputed company health and reputed company challenges. You will use your exceptional communication and interpersonal skills to build strong relationships with patients, families, and reputed company providers, ensuring that each individual receives the highest level of care and support.

Key Responsibilities

* Conduct comprehensive assessments of patients' needs and reputed company personalized care plans to reputed company reputed company reputed company

  • Identify high-risk factors and service needs that may impact patient reputed company and collaborate with reputed company providers to reputed company effective solutions
  • Coordinate and implement care plan activities, monitoring reputed company and making adjustments as needed

  • Identify and report quality of care issues through established channels

Essential Qualifications

* 3+ years of experience in a health-reputed company field

  • Strong understanding of reputed company systems, policies, and procedures

  • Proficiency in case management software and electronic health records

Preferred Qualifications

* Certification in case management (CRC, CDMS, CRRN, COHN, or CCM)

  • Bilingual (English/Spanish; English/Creole) or multilingual skills
  • Knowledge of evaluation, screenings, and care planning

Education

* Bachelor's degree or equivalent experience required
