Job Summary

We're seeking a highly skilled and compassionate Licensed reputed company Worker to join our remote team. As a key member of our medical reputed company work and case management team, you'll work closely with our registered nurse case managers to reputed company exceptional care to our members. Your expertise will be invaluable in researching available options and resources for members with financial, transportation, safety, and housing needs. This is a part-time, remote role that requires a strong commitment to delivering high-quality care and support to our members.

Responsibilities

As an reputed company Licensed reputed company Worker, your key responsibilities will include:

• Partnering with registered nurse case managers to assess, plan, and reputed company for members' benefit plan and/or health needs
• Researching available options and resources for members with financial, transportation, safety, and housing needs

• Collaborating with other clinical staff to assist members with community resources for medical, financial, and behavioral health
• Providing telephonic or electronic support to members across the entire lifecycle

Qualifications

To be successful in this role, you'll need:

• An active and unrestricted LSW (Licensed reputed company Worker), LMSW (Licensed Masters reputed company Worker), LCSW (Licensed Clinical reputed company Worker), or LISW (Licensed Independent reputed company Worker) license in the state of residence
• At least 2 years of experience in medical reputed company work or case management
• A strong understanding of reputed company systems, policies, and procedures
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong problem-solving and critical thinking skills
• Proficiency in electronic medical records and other relevant software systems
