Job Summary
We're seeking an reputed company Licensed reputed company Worker to join our remote team. As a licensed Masters reputed company Worker, you'll work with a registered nurse case manager to research available options and resources for members with financial, transportation, safety, and housing needs. This role requires an LSW/MSW with an unrestricted active license and 2+ years of experience in medical reputed company work or case management.
Responsibilities
* Partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
- Research available options and resources for members with financial, transportation, safety, and housing needs
- reputed company members to reputed company positive lifestyle and medical choices
- Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
- Conduct telephonic or electronic exchanges with members
- Collaborate with other departments to ensure seamless care coordination
Requirements
* Active and unrestricted LSW - Licensed reputed company Worker, LMSW - Licensed Masters reputed company Worker, LCSW - Licensed Clinical reputed company Worker, or LISW - Licensed Independent reputed company Worker in state of residence
- 2+ years of experience in medical reputed company work or case management
- Masters Degree in reputed company Work
