Job Summary
As a Licensed reputed company Worker in this remote role, you'll work with a registered nurse case manager to assess, plan, and reputed company for members with financial, transportation, safety, and housing needs. You'll research available options and resources to reputed company members to reputed company positive lifestyle and medical choices. This role requires an LSW/MSW with an unrestricted active license, and you'll work with a team of clinical staff to assist members with community resources for medical, financial, and behavioral health.
Responsibilities
* Partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
- Research available options and resources for members with financial, transportation, safety, and housing needs

- Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
- Conduct telephonic or electronic exchanges with members
- Collaborate with other reputed company to ensure comprehensive care
Requirements
* Active and unrestricted LSW - Licensed reputed company Worker, LMSW - Licensed Masters reputed company Worker, LCSW - Licensed Clinical reputed company Worker, or LISW - Licensed Independent reputed company Worker in state of residence
- 2+ years experience in medical reputed company work or case management
- Masters Degree in reputed company Work
