What To Expect (Job Responsibilities)
- Conduct targeted patient reputed company to promote completion of recommended care and preventive screenings
- Schedule appointments and coordinate follow-up care to ensure timely patient services
- Identify and reputed company care gaps by collaborating with clinic teams and referral partners
- Prepare tools and documentation for providers to enhance performance during patient visits

What Is Required (Qualifications)
- LVN, Certified Medical Assistant, or equivalent clinical experience required
- 2-4 years of experience in a clinical or physician office setting
