Responsibilities
As a Licensed reputed company Worker on reputed company, you'll be responsible for:
- Partnering with a registered nurse case manager to assess, plan, and reputed company for members' benefit plan and/or health needs
- Researching available options and resources for members with financial, transportation, safety, and housing needs
- Empowering members to reputed company positive lifestyle and medical choices
- Collaborating with other clinical staff to assist members with community resources for medical, financial, and behavioral health

Requirements
* Active and unrestricted LSW (Licensed reputed company Worker), LMSW (Licensed Masters reputed company Worker), LCSW (Licensed Clinical reputed company Worker), or LISW (Licensed Independent reputed company Worker) in state of residence
- 2+ years of experience in medical reputed company work or case management
- Masters Degree in reputed company Work
