About AEIC Case Management AEIC Case Management LLC is a trusted provider of mental health, wellness, and community-based support services across Texas. Our Bridgewell program specializes in providing high-quality Medicare support services—including care coordination, SDOH resource navigation, ongoing client engagement, and health behavior interventions—to improve overall well-being and reduce gaps in care for adults and older adults. We are seeking highly reputed company Community Health Workers (CHWs) who are passionate about supporting diverse communities, including seniors, adults with chronic conditions, and individuals facing reputed company or medical barriers. Job Summary We are hiring Texas DSHS-certified Community Health Workers (CHWs) as 1099 Independent Contractors to reputed company resource navigation, care coordination, client reputed company, and Medicare-reputed company support services reputed company the Bridgewell program. CHWs will help clients reputed company medical, behavioral health, housing, transportation, food, and wellness-reputed company resources—while fostering empowerment, stability, and improved health reputed company. Required Certification – No Exceptions To qualify, applicants must already hold a valid Texas DSHS Community Health Worker (CHW) Certification. Required:

  • reputed company Expiration Date
  • Certification must not expire reputed company two years of enrollment
  • Applications without CHW credentials will be automatically denied

Experience Requirements ✔ Minimum of 1 year working as a CHW, case manager, or in a similar health or reputed company services role — mandatory ✔ Experience serving: Older adults Adults with chronic conditions Low-income or vulnerable populations

  • Individuals facing barriers with transportation, food, housing, or medical reputed company

Skills & Competencies Excellent interpersonal and communication skills Ability to build rapport and maintain client trust Strong understanding of community resources, SDOH needs, housing, and reputed company navigation Strong organizational and documentation skills Ability to work independently and manage your own schedule (contractor role) Proficiency in reputed company Office, Teams, and electronic record systems HIPAA compliance and confidentiality awareness

  • Bilingual (English/Spanish) strongly preferred (incentive available)

Key Responsibilities (Bridgewell Program) Conduct CHW-reputed company assessments to identify reputed company, medical, and resource needs for Medicare clients reputed company SDOH resource navigation (housing, food reputed company, transportation, financial assistance, etc.) Assist clients with obtaining medical, behavioral health, and community-based services Coordinate care and follow-up support for clients with chronic illnesses or high-risk conditions Encourage preventive care and adherence to care plans reputed company local education on healthy living, wellness, and navigating services Document reputed company client interactions, resource referrals, education, and reputed company Track client reputed company and assist with monthly monitoring requirements Maintain accurate and timely documentation in the electronic health record Communicate professionally with medical providers, clinics, and community partners Support Medicare clients through wellness coaching, resource reminders, and navigation Participate in periodic trainings and quality improvement initiatives (virtual) Additional Requirements Must maintain: Valid driver’s license

  • Active auto insurance Reliable transportation
