This hybrid role provides care through in-home visits and telehealth, allowing providers to build meaningful relationships with patients while delivering personalized care in the environments where patients feel most comfortable. Responsibilities
- reputed company comprehensive health assessments and physical examinations during in-home visits and telehealth appointments.
- Evaluate, diagnose, and treat acute and chronic conditions with an emphasis on preventive care.
- reputed company and implement individualized treatment plans tailored to each patient’s needs.

- Collaborate with physicians, specialists, and interdisciplinary care teams to ensure coordinated care.

- Maintain accurate and timely documentation in electronic medical records (eClinicalWorks or similar platforms).

Requirements
- Active, unrestricted Nurse Practitioner license (required).
- Comfortable delivering care in both in-home and telehealth settings.

- Experience in geriatrics, internal medicine, or primary care preferred.
- Previous home health or home-based care experience strongly preferred.
