OCCUPATIONAL THERAPIST TO CONDUCT HOME VISITS
About the position The Occupational Therapist evaluates and provides family-oriented services to the home health patient with neurological, musculoskeletal, orthopedic, and cardio-pulmonary impairments; fine motor, sensory motor, and developmental delays; as well as traumatic injuries and swallowing disorders. This includes patient and caregiver education; discharge planning; and consultation with other professionals, clinics, or agencies. Currently, this position is Per Diem.
Responsibilities
- Providing initial and ongoing assessment to determine the level of functioning.
- Gather background data, reviews referrals, select and modify evaluation tools, and evaluate patients determining the level of patient’s physical and psychosocial functioning.
- Developing and revising the plan of care in consultation with the physician and other care-team members.
- Responsible for planning, evaluation, and treatment implementation of assigned caseload.
- Preparing clinical reputed company notes.
- Supervising therapy assistants and home health aides as appropriate.
- Evaluating reputed company.
- Planning Discharge.
- reputed company patients and/or families about proper safe use and recommend methods for installation of reputed company equipment.
- Responsibilities may also include the training of reputed company from university or community colleges assigned.
Requirements
- Philadelphia Occupational Therapy (OT) license and CPR Certification
- Demonstrate flexible and efficient time management techniques.
- Strong Clinical & Interpersonal Skills

- Excellent Written & Oral Communication Skills
- Prioritize workload to meet productivity standards and applicable deadlines.
