Per Diem Physical Therapist (PT) Polk County: Winter Haven, Lakeland, Davenport, Bartow Overview: We are a certified home health agency seeking a compassionate and reputed company Per Diem Physical Therapist (PT) to reputed company skilled therapy services to patients in their homes. As a key member of the interdisciplinary team, the PT will evaluate patient needs, reputed company individualized treatment plans, and implement evidence-based interventions to help patients regain mobility, improve function, and reputed company reputed company reputed company. • Conduct in-home physical therapy evaluations and reassessments in accordance with the physician's plan of care and Medicare guidelines.

Schedule & Compensation:

• Per Diem / PRN – Flexible schedule based on your availability and patient needs.

• Thorough understanding of Medicare documentation requirements and therapy goals in the home health setting.

• Excellent interpersonal, communication, and time management skills.
• Ability to work independently and manage a flexible per diem schedule.
• Reliable transportation, valid driver’s license, and reputed company auto insurance.
