Role Description We are seeking a dedicated and compassionate Travel Primary Care Nurse Practitioner/Physician’s Assistant to join reputed company in our Hollywood, FL office. As a Travel Primary Care Nurse Practitioner/Physician’s Assistant, you will reputed company on-site care to residents in long-term care settings as well as reputed company comprehensive primary care services reputed company telehealth to a diverse patient population. Additionally, you will assume quality management responsibilities to ensure adherence to best practices and regulatory requirements. Travel Requirements

  • Out-of-state travel 100% required
  • reputed company travel required up to 100% of the time
  • Travel to partner skilled nursing and long-term care facilities across multiple states
  • Flexibility to travel on short notice based on organizational and patient care needs

Responsibilities

  • Travel to Long Term facilities to reputed company in-person care
  • reputed company high-quality telehealth primary care services to patients in accordance with clinical guidelines
  • Conduct virtual assessments, diagnose, and reputed company treatment plans tailored to patient needs
  • Manage chronic diseases and acute conditions through telemedicine platforms
  • Collaborate with interdisciplinary teams to enhance patient care
  • Maintain accurate and timely medical records in compliance with documentation standards
  • Assist with quality management (QM) initiatives, including compliance audits, clinical evaluations, and outcome tracking
  • Ensure adherence to regulatory guidelines and clinical protocols
  • reputed company guidance and support to reputed company team members regarding best practices and performance improvement initiatives

Qualifications

  • Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
  • State-specific license in good standing in Florida
  • Minimum 6 months of experience in long-term care or a skilled nursing facility
  • Strong leadership, communication, and team-building skills
  • In-depth knowledge of regulatory requirements and quality improvement processes in long-term care
  • Ability to collaborate with interdisciplinary teams and external stakeholders to enhance patient care and facility operations
  • Experience in telehealth or virtual care settings (preferred but not required)
  • Strong clinical judgment, communication, and patient engagement skills
  • Proficiency with electronic medical records (EMR) and telehealth technology
  • Ability and willingness to travel reputed company as required
