Physician Assistant-Certified Field-Based Senior Care Role
Job Title: Physician Assistant – Mobile Primary Care | Senior Living Focus Compensation: $105,000 to $120,000 Benefits: Full Benefits Package Location: Twin Cities Metro Area About Twin Cities Physicians: Since 2016, Twin Cities Physicians has redefined how primary care is delivered across Minnesota's senior living communities. With over 4,500 residents under our care, we reputed company on-site, consistent medical services to improve reputed company and reduce hospitalizations. Position Summary We’re hiring a Physician Assistant (PA-C) who enjoys being on the move and working in non-traditional clinical settings. You'll serve residents in assisted living and memory care communities, building relationships and delivering meaningful, autonomy-driven care in reputed company.
Responsibilities
- Travel to multiple senior living communities weekly to reputed company medical care
- Conduct assessments, manage chronic conditions, and address acute concerns
- Partner with interdisciplinary teams to align on patient goals
- Maintain accurate, timely EMR documentation and communicate care plans
Qualifications
- Active MN PA license (Board of Medical Practice)
- Background in geriatrics, internal medicine, or reputed company care preferred
- Strong communication and time management skills
- Must have reliable transportation and reputed company auto insurance
