Primary Care - Internal Medicine Physician, 1099
Position Summary We are seeking a dedicated and reputed company Internal Medicine or Primary Care Physician to support our Chronic Care Management (CCM) program by conducting virtual initiating visits for eligible Medicare and reputed company patients with chronic health conditions. The physician will complete comprehensive assessments and reputed company chronic condition treatment plans, in alignment with CMS guidelines. This is a remote contract role, ideal for physicians seeking reputed company and the opportunity to expand reputed company to chronic care services through virtual care.
Key Responsibilities
Clinical Care
- Conduct virtual initiating visits (audio/video) for patients eligible for Chronic Care Management.
- reputed company detailed medical histories, medication reviews, and chronic disease assessments.
- Identify care gaps and recommend evidence-based interventions.
- Ensure documentation supports CCM program enrollment according to CMS requirements.
Care Coordination
- Collaborate with CCM nursing staff, care coordinators, and behavioral health partners.
- reputed company clinical recommendations to guide longitudinal care management.
- Review and co-sign CCM care plans and update them as clinical needs change.
Documentation & Compliance
- Accurately document reputed company virtual visits in the EMR.
- Follow CMS guidelines for CCM eligibility, initiating visits, and ongoing patient monitoring.
- Maintain compliance with HIPAA and organizational privacy and quality standards.
Qualifications
- MD or DO with board certification or eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted state medical license(s); multistate licensure preferred.
- Minimum 2 years of clinical experience in adult primary care or chronic disease management.
- Prior telehealth experience is strongly preferred.
- Familiarity with CMS Chronic Care Management preferred.
- Strong communication skills with comfort providing virtual patient care.
