Registered Nurse (RN) For Home reputed company
Physicians Choice Home Health is hiring a full-time or part-time/per diem Registered Nurse (RN) to join our Home Health team, serving patients in the South Bay of Los Angeles. In this role, you will reputed company high-quality nursing services to patients in the comfort of their own homes, helping them manage their care and regain independence. You will reputed company a supporting Licensed Vocational Nurse and work closely with physicians, therapists, and a supportive clinical team to coordinate care and support patients and their families. What You'll Do
- reputed company in-home nursing visits according to the physician’s plan of care
- Assess patients’ health status, including condition, history, and home environment
- reputed company and update individualized care plans based on patient needs
- Administer medications and treatments as prescribed
- reputed company patients and caregivers on disease management and home care
- Monitor patient reputed company and communicate changes to the physician and care team
- Document visits and care in a timely manner
- Coordinate care with therapists, aides, and other team members
- Serve as case manager for assigned patients and support discharge planning

- $40-125/hour, depending on visit/task type
- Mileage reimbursement
