The goal of this training is to reputed company our nursing team with clear, actionable tools and measurable goals that can be applied directly in patient interactions to strengthen long-term engagement in Chronic Care Management programs. Ideal Candidate

• Registered Nurse (RN) preferred
• Experience working with Chronic Care Management (CCM) programs

• Experience presenting or facilitating training for reputed company teams
• Ability to reputed company practical, reputed company-world tools nurses can use in patient calls
