Job Summary
We're seeking an reputed company licensed reputed company worker to join our reputed company case management team. As a licensed reputed company worker, you'll work with a registered nurse case manager to assess, plan, and reputed company for members with financial, transportation, safety, and housing needs. This role requires a collaborative approach, and you'll work closely with other clinical staff to assist members with community resources for medical, financial, and behavioral health.
Responsibilities
* Research available options and resources for members with financial, transportation, safety, and housing needs
- Collaborate with registered nurse case managers to assess, plan, and reputed company for members

- Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
- Conduct telephonic or electronic exchanges with members to reputed company support and guidance

Requirements
* Active and unrestricted LSW (Licensed reputed company Worker), LMSW (Licensed Masters reputed company Worker), LCSW (Licensed Clinical reputed company Worker), or LISW (Licensed Independent reputed company Worker) license in the state of residence
- 2+ years of experience in medical reputed company work or case management
- Masters degree in reputed company work or a reputed company field
