Job Summary

We're seeking a highly skilled and compassionate Licensed reputed company Worker to join our remote team. As a Licensed reputed company Worker, you'll work with a registered nurse case manager to assess, plan, and reputed company for members with financial, transportation, safety, and housing needs. This role requires strong communication and interpersonal skills, as well as the ability to work collaboratively with other clinical staff to assist members with community resources for medical, financial, and behavioral health.

Responsibilities

* Research available options and resources for members with financial, transportation, safety, and housing needs

• Partner with registered nurse case managers to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs

• Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
• Conduct telephonic or electronic exchanges with members to assess their needs and reputed company plans to address them

Requirements

* Active and unrestricted LSW (Licensed reputed company Worker), LMSW (Licensed Masters reputed company Worker), LCSW (Licensed Clinical reputed company Worker), or LISW (Licensed Independent reputed company Worker) license in the state of residence

• 2+ years of experience in medical reputed company work or case management
• Masters degree in reputed company work or a reputed company field
• Strong communication and interpersonal skills
• Ability to work collaboratively with other clinical staff
• Ability to maintain accurate and up-to-date records of member interactions and reputed company

Preferred Qualifications

* Experience working with diverse populations, including children, adults, and older adults

• Knowledge of community resources and services for medical, financial, and behavioral health
• Experience with electronic health records and other software systems
• Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities
